Recently I attended a confined space course that used an LODD incident that occurred at a naval base in Virginia as a case study. The incident was a confined space rescue that occurred at the Little Creek amphib base sometime in the late 80's (1988 I believe). The incident involved the death of a fire captain who had entered the hold of a ship to rescue a worker. I believe the captain worked for the fire department on the base but I am not sure. This case study kind of surprised me because I thought I was fairly familiar with most of the Virginia LODDs and I had never heard of this one. I have tried to research more about the details of the incident through the internet but so far I haven't had much luck. Even the NIOSH website doesn't have a record of the incident. If anybody is familiar with the incident I would appreciate any information you could provide.

Yes, this event happened. If you call Little creek they should have the info. Their is even a video that was made. I think they used VBfirefighters. They maybe a place to contact.
I have seen the video. I'm not sure if my present department has one. I will look. Here is the union web site for the navy in that area. They may have the info your looking for.http://tff25.org/

Good luck on your search.
